INSTRUCTIONS FOR COMPLETING A SIGNON PROFILE UPDATE REQUEST FOR TRANSACTIONS
The Date Needed field is used to inform the person who processes the form
as to when you need the update completed. There is a one day minimum turnaround after the completed/approved
form has been received by the person who processes the form.
The Principal User Name, Dept, Dept #, Phone and Date must all be
filled in or the form will be rejected.
The Dept # field has been added. This field as well as the Date
must both be filled in.
The Principal User Signon Name has been shortened to a maximum of
seven characters. (Only one signon name per form, no exceptions!)
The Add and Del fields describe what action is to be performed
with the transaction field.
The Transaction field is the four character transaction id of the
transaction you wish to have access to. (Only one transaction id per line. Use multiple
forms if necessary.)
The Approved by Custodian of Transaction field has been clarified.
This field is to be used only by the Fiscal agent who funded the development of the transaction. (The
Custodian of the transaction.) The signature on this line gives the
Principal user permission to use the transaction.
The Confidentiality Statement should be read and understood by the
Principal User before signing the Principal User Signature line.
The Principal User Signature field is to be signed by the Principal
user of the Signon Name above, after reading and understanding the Confidentiality Statement.
The Supervisor signature field is to be signed by the Supervisor of the
Principal user who has signed above.
The If questions call field has been added. This field should be
filled in with the name and Phone number of the person who has completed the form.(Please print the name)
The Comments field is for any additional comments from the user to
the person who processes the form.
