INSTRUCTIONS FOR COMPLETING A NVAS SIGNON UPDATE REQUEST

         The Date Needed field is used to inform the person who processes the form as to when you need the update completed.  There is a one day minimum turnaround after the completed/approved form has been received by the person who processes the form.

         The next section of the form lists the possible requests.  Place an X next to the type of signon update request required.

         The NVAS Name must be your first initial, middle initial and the first 5 characters of your last name.  This field is required for all options.  Only one signon name per form, no exceptions!

         For Principal user and Supervisor the fields: Name, Deptartment, Deptartment #, UID and Phone must all be filled in.

         The Use with project number field is part of the security system that only allows the signon to use specific project number when signing on.

         The Use with PA1 printer field is the 4-character CICS printer name.

         The First transaction started after signon field specifies a CICS transaction that can be invoked once the signon process is complete.

         The Principal user signature field is to be signed by the Principal user.

         The Supervisor signature field is to be signed by the Supervisor of the Principal user.

         The Comments field is for any additional comments from the principal user to the person who processes the form.

         The If questions call field has been added.  This field should be filled in with the name and Phone number of the person who has completed the form.(Please print the name)
